Flexible Schedule | Community-Based Role | Work-Life BalancePosition: Licensed Practical Nurse (LPN) – In-Home & Telehealth Support

Join Anpex Health and be part of an innovative, patient-centered team delivering high-quality care directly in patients’ homes while collaborating with a Nurse Practitioner-led virtual care model.

Essential Functions
  • Conduct comprehensive in-home patient assessments, including vital signs, medication reconciliation, and focused physical observations

  • Assist with and facilitate Nurse Practitioner (NP)-led telehealth visits using mobile devices and telehealth platforms

  • Communicate clinical findings clearly and accurately to the NP and interdisciplinary care team

  • Complete timely and accurate documentation in the electronic medical record (EMR)

  • Identify and report changes in patient condition or safety concerns

  • Provide patient education and reinforce care plans as directed by the NP

  • Support care coordination and continuity between in-home visits and virtual care

  • Ensure compliance with clinical protocols, company policies, and HIPAA regulations

Education & Experience
  • Active Licensed Practical Nurse (LPN) license in good standing

  • Experience in home care, geriatrics, or primary care preferred, but not required

  • Training provided for telehealth workflows and in-home assessment model
