Job description

RN Supervisor – Home Health (Pediatric) | Flexible Schedule | High Pay Per Visit (Field-Based) 

IntegriCare is hiring an experienced RN Supervisor to oversee care for medically fragile pediatric clients in the home setting. This is a flexible, field-based role ideal for nurses with strong clinical and supervisory experience.

Compensation
• $100 per Nursing Assessment Visit
• $70 per Supervisor Visit
• $0.60/mile reimbursement for travel 30+ miles outside Atlanta

Case Load

  • Territory is Atlanta and South Georgia
  • Case Load may be 15+ clients 
  • Up to 15+ Supervisor Visits per month (virtual visits allowed sometimes But in-person visits required)
  • 5+ RN assessments required monthly (in-person visits required for all assessments)

Key Responsibilities

Clinical Oversight & Patient Care
• Conduct comprehensive in-home nursing assessments and reassessments
• Conduct initial face-to-face assessments in the client’s home
• Admit new clients and complete all required clinical documentation
• Develop, review, and revise Plans of Care in collaboration with physicians
• Ensure care aligns with physician orders and Medicaid guidelines
• Monitor medication administration, pain management, and patient condition
• Evaluate home environments and recommend safety improvements

Supervision & Staff Management
• Supervise and train LPNs, CNAs, PCAs, Companions, and Sitters
• Perform bi-weekly supervisory visits for non-medical staff and monthly visits for LPNs
• Conduct annual staff evaluations through direct observation or demonstration
• Assist with orientation and ongoing training of caregivers
• Ensure caregivers are following proper care procedures and maintaining quality standards

Compliance & Quality Assurance
• Ensure compliance with GAPP, Medicaid, and Department of Community Health regulations
• Complete all required supervisory documentation accurately and on time
• Review all Plans of Treatment for accuracy, appropriateness, and completeness
• Investigate complaints and conduct follow-up supervisory visits as needed
• Participate in quality improvement initiatives

Coordination & Communication
• Participate in establishing and implementing client service plans
• Regularly assess and reassess client nursing needs
• Communicate client progress, concerns, and updates to physicians and internal staff
• Coordinate care between families, caregivers, and clinical teams


Essential Functions
• Participate in establishing and implementing the client’s service plan with care staff
• Regularly assess the nursing needs of each client
• Conduct initial and ongoing in-home, face-to-face assessments
• Assess home environments to determine care needs and safety risks
• Report client progress, concerns, and issues to supervisors and physicians
• Perform annual evaluations of LPNs, PCAs, CNAs, Companions, and Sitters
• Conduct supervisory visits in response to complaints or service concerns
• Review all Plans of Treatment for accuracy and completeness
• Assist with orientation and annual evaluations of caregivers
• Perform supervisory visits to observe caregivers, provide guidance, and ensure client satisfaction


Minimum Education & Experience Requirements
• Bachelor’s degree in Nursing or related field
• Active Registered Nurse (RN) license in the state of Georgia
• Minimum 2+ years of medical supervisory experience
• Experience managing a caseload of 5+ clients preferred
• Home healthcare experience required
GAPP (Georgia Pediatric Program) experience strongly preferred
• Valid driver’s license and reliable vehicle
• Strong organizational, clinical, and communication skills
• Must be tech-savvy (EMR systems, documentation platforms, digital communication tools)


Schedule Expectation
This is a part-time flexible, field-based role, however it will require up to ~20 hours per week including driving, documentation, and communication to effectively manage your caseload.
Scheduling is determined based on client needs and agency priorities.
