Job description

Job Summary: As a Chaplain of Kindful Health you provide spiritual support, guidance, and resources to patients and their families. The Chaplain provides compassionate services to a diverse population including all faiths that are represented by patients, families, and team members, while at the same time, reflecting the mission and philosophy of Kindful.

Essential Education and Experience: Graduate of accredited seminary or school of theology, or appropriate certification in pastoral ministry. Minimum one (1) year experience as a healthcare pastor/chaplain. One (1) year of hospice experience preferred but not required.

Responsibilities & Duties: The Chaplain will be responsible for:

  • Adheres to all regulatory requirements and professional standards
  • Assesses patient/family to identify spiritual needs and develops a spiritual plan of care.
  • Collaborates with local clergy as needed.
  • Conducts services of prayer, worship and ritual for patients/families, as appropriate.
  • Provides individual patient/family counseling, crisis intervention, information and assistance with preparation of advanced directives, funeral planning issues and transfer of responsibilities relative to legal and fiscal issues.
  • Work with IDT, participates in the ongoing POC by developing, documenting and evaluating responses to interventions in order to meet spiritual needs of the patient and family.
  • Provides consultation, education, and support to the interdisciplinary team on spiritual care and bereavement counseling.
  • Makes visits to patient/family prior to death and will be available to family during the bereavement period as needed.
  • Follows policies and procedures consistent with recognized standards of professional practice
  • Provide high levels of communication both internally with the Kindful team and externally with patients and families to assure top CAHPS scores
  • Collaborate with your Clinical Director and entire team to provide a "concierge" level hospice experience

Compensation:

Salary $60,000 - $75,000
