JOB DESCRIPTION:
SUMMARY OF POSITION
Provides services that help consumers gain access to needed medical, social, educational and other services. This includes direct assistance in gaining access to services, coordination of care, oversight of the entire case, linkage to appropriate services, community-based work and ensuring continuation of entitlements.
ESSENTIAL DUTIES & RESPONSIBILITIES
  • Provides direct client services. Manages case load of life skills training for clients.
  • Facilitates client's use of family, social and community resources, acting as an advocate when necessary and maintaining necessary communication with other involved parties.
  • Assesses and develops person-centered plan and monitor progress of the plan.
  • Collaborates with team and outside agencies. Serves as the main point of connect for communication with resources or other entities to ensure continuum of care.
  • Completes client assessments (ex: Ansa, PHQ9, GAD7) and develop care plans, monitor client's progress through care plan.

KNOWLEDGE, SKILLS & ABILITIES
  • Effectively communicate via written, verbal, in person and virtual methods.
  • Customer service skills.
  • Time management and organization skills.
  • Analytical thinking skills.
  • Therapeutic skills and knowledge of EBPs.

QUALIFICATIONS
Education Level
Bachelor's degree from a four-year college or university; preferably in psychology, social work, or a related field.
Years of Experience
2 years of experience is preferred.
