ABOUT THE JOB
Collaborate with providers to create, manage and maintain patient care plans, including self-management activities, leveraging the appropriate care management resources from Marathon Health services, the health plan, the community, and other employer-based resources. In addition, this position is responsible for a wide variety of tasks involving clerical and administrative functions for providing support to Anywhere providers. As a key member of the clinical team, this role is on the front lines in supporting the company vision and exemplifies organizational core values. Strong customer service skills, accompanied by exceptional communication and computer skills, are key ingredients to success in this position. This position requires flexibility and adaptability in performance of duties as needed, to assure consistent and seamless high quality customer service and clinical excellence.
ESSENTIAL DUTIES & RESPONSIBILITIES

  • Performs nursing assessments and triage of patient care needs for new and ongoing members, including virtual care patients and in-person clinics.
  • Support member check-out (as needed), triage, scrubbing schedule specifically for compliance with state regulatory/ licensure compliance (day before visits), internal care team referral management, billing support, administrative support, member calls for labs/imaging, member messages and questions, medication refills, pharmacy questions, etc.
  • Work with members that call in to schedule a visit, need help with mobile app to schedule or with the portal to schedule. Answers basic member questions regarding health promotion initiatives or connects to resources that can assist.
  • For those individuals with an existing specialist or previous specialty care, and in particular for those individuals who have multiple complicated medical conditions requiring specialty care and/or significant oversight, the clinician will work in collaboration with said provider to provide adjunct care and education to the member.
  • Assist providers with all aspects of appropriate virtual patient care, such as assisting with referrals, scheduling labs, managing prior authorizations, entering information into the EMR, contacting patients, etc.
  • For times when Physicians cannot "lay hands" on the patient and an evaluation is required, the CCC will need to arrange for the external patient visit and coordinate results.
  • Perform medical record requests and abstraction for virtual care patients. Document virtual care interactions accurately within EMR.
  • Process test results and provide the participant with timely results via e-message, mail or phone as directed in written instructions by the Provider.
  • Outreach to participants to drive engagement among eligible virtual care groups.
  • Understanding the client's current benefits programs for member assistance
  • Assist members to understand their available employer benefits solutions as appropriate
  • Work with members to identify and remove barriers created by social determinants of health
  • Knowledgeable of community resources such as housing, food, transportation and childcare.
  • Provide assistance in finding cost affordable options for medications, outside care, transportation, groceries, housing, and other SDH issues when needed.
QUALIFICATIONS
Completion of ASN or BSN with RN licensure, preferrable. Training as a Registered Nurse is required (may consider LPN with extensive experience). Must be experienced with patient triage and can adequately use an EMR system. Ability to navigate web-based platforms to generate reports, order labs, schedule appointments, etc.

Pay Range: $70,000 - $90,000/yr
