Job Description
To provide the highest quality of care to Accord Services clients utilizing the Plan of Care provided to ensure client's needs are met in relation to safety, well-being and satisfaction.
ESSENTIAL JOB RESPONSIBILITIES:
โ€ข Provide medical and personal care for the client based on individualized Plan of Care prepared by Nurse Supervisor available in the client's home resulting in satisfaction for all parties involved
โ€ข Orient new caregivers to cases and clients' plan of care resulting in clients' clinical needs being met
โ€ข Provide prompt communication of changes in client medical status/emergencies to Nurse Supervisor resulting in effective management of cases
โ€ข Provide prompt communication of changes in clients/caregivers schedules (i.e. tardiness, vacation, requested time-off, change of status, reduction in hours, etc.) to Staffing Coordinators resulting in effective management of cases
โ€ข To drive/escort client to and from appointments/activities as requested, utilizing client vehicle, resulting in a safe and timely transport
โ€ข Call in to Staffing Coordinator to confirm weekly schedules resulting in required coverage being met
โ€ข Provide accurate and timely required documentation (i.e. declining to participate in all parts of Plan of Care) ensuring effective management of cases and DHR compliance
โ€ข Other duties as assigned or needed
Qualifications
LPN requirements:
  • High school diploma/GED
  • CPR certification
  • Current TB skin test or Chest X-ray
  • Certificate from accredited nursing program
  • valid drivers license (if assigned to a case with driving privileges)
  • Previous experience is preferred, but not required
