Practice Support Specialist – ValueBased Care

Schedule: Fulltime Days - Monday - Friday
Work Environment: Fully Remote to Alabama Residents Only (with occasional inoffice team meetings)

As a Practice Support Specialist, you’ll play a key part in ensuring patients have a smooth, informed, and positive care experience. What You’ll Do

  • Review electronic health records to prepare for patient visits
  • Complete previsit outreach, wellness questionnaires, and patient history reviews
  • Gather and update patient demographic and health information
  • Conduct medication reviews and assist care teams with accurate documentation
  • Obtain health maintenance documents and ensure proper record workflows
  • Reach out to patients with gaps in care and help them schedule needed appointments
  • Educate patients and families on prevention, wellness, and followup care
  • Collaborate with care management, providers, riskadjustment coders, and community partners
  • Participate in team meetings and organizational quality initiatives

What You Bring

  • High school diploma or equivalent
  • Medical Assistant training from an approved technical school (required)
  • Current Medical Assistant certification or an active LPN license (Alabama)
  • At least one (3) year of experience in a primary care practice or valuebased care setting
