What You’ll Do
- Review electronic health records to prepare for patient visits
- Complete previsit outreach, wellness questionnaires, and patient history reviews
- Gather and update patient demographic and health information
- Conduct medication reviews and assist care teams with accurate documentation
- Obtain health maintenance documents and ensure proper record workflows
- Reach out to patients with gaps in care and help them schedule needed appointments
- Educate patients and families on prevention, wellness, and followup care
- Collaborate with care management, providers, riskadjustment coders, and community partners
- Participate in team meetings and organizational quality initiatives

What You Bring
- High school diploma or equivalent
- Medical Assistant training from an approved technical school (required)
- Current Medical Assistant certification or an active LPN license (Alabama)
- At least one (3) year of experience in a primary care practice or valuebased care setting

- Ability to work independently and collaboratively
