As a Practice Support Specialist, you’ll play a key part in ensuring patients have a smooth, informed, and positive care experience. Working as part of a multidisciplinary team, you’ll help drive critical valuebased care initiatives by supporting providers, improving care coordination, and helping patients close gaps in care.

ALL BENEFITS START DAY ONE

• Review electronic health records to prepare for patient visits
• Complete previsit outreach, wellness questionnaires, and patient history reviews
• Gather and update patient demographic and health information
• Conduct medication reviews and assist care teams with accurate documentation
• Obtain health maintenance documents and ensure proper record workflows
• Reach out to patients with gaps in care and help them schedule needed appointments
• Educate patients and families on prevention, wellness, and followup care
• Collaborate with care management, providers, riskadjustment coders, and community partners
• Participate in team meetings and organizational quality initiatives

Why You’ll Love Working With Us

• High school diploma or equivalent
• Medical Assistant training from an approved technical school (required)
• Current Medical Assistant certification or an active LPN license (Alabama)
• At least one (3) year of experience in a primary care practice or valuebased care setting
• Strong communication skills, attention to detail, and a patientcentered approach
• Ability to work independently and collaboratively
