As a Home Health MSW, hereโ€™s a big-picture view of what youโ€™ll do:

  • Provide home-based social services to patients dealing with depression, anxiety, poor nutrition or a lackย of resources that make managing their injury or disease difficult
  • ย Work as part of a home health team which may include an RN, LPN, CNA, HHA, PT, OT or SLP, focused on the patientโ€™s plan of care and individual goals
  • Monitor a patientโ€™sย psychosocial condition and identify social and emotional needs
  • Conduct patient assessments, document progress and ensure patient is moving toward goals
  • Provide counseling, community resource planning, crisis intervention and advocacy Consult with family and caregivers on patientโ€™s plan of care and how to help them progress

ย A few must-haves for Home Health Medical Social Workers:

  • Masterโ€™s degree in Social Work
  • Active MSW license in Virginia preferred
