POSITION SUMMARY

The Veteran’s Grant Manager is responsible for overseeing data collection the Veteran’s Program, managing client consensus, providing case management, and clinical services and support.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Essential functions include but are not limited to the following:

  • Tracks daily census of veterans and maintains daily roster of current clients
  • Maintains daily per diem billing
  • Tracks and analyzes data and outcomes
  • Maintains proper documentation in client files
  • Maintains complains with VA requirements and inspections
  • Works with VA liaison to receive and make veteran referrals to Low Demand and Clinical Treatment Programs
  • Partners with VA liaison on veteran service eligibility, housing and goal progress
  • Works with Coalition for the Homeless to enroll clients in housing programs and/or monitor progress in housing
  • Provides case management services as needed
  • Conducts psychoeducation classes
  • Coordinates with other departments within The Healing Place as needed
  • Performs outreach services to homeless veterans
  • Transports veterans to appointments as needed
  • Participates in VA case conferences and technical calls
  • Responds to emergency requests providing crisis intervention services as appropriate
QUALIFICATIONS / REQUIREMENTS
  • High school diploma or equivalent. Bachelor’s degree strongly preferred.
  • CADC preferred or ability to obtain CASC in Kentucky required
  • Working knowledge of Microsoft Works Suite required (Outlook, Word, Excel)
  • Valid Driver’s License required
