• Conduct comprehensive clinical assessments via telehealth platforms to evaluate patients' mental health status and needs.
• Develop individualized treatment plans utilizing evidence-based practices and solution-focused interventions tailored to the unique challenges faced by patients in hospice care.
• Deliver therapeutic interventions including cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT) while adapting techniques specifically for a remote delivery format.
• Maintain accurate and timely clinical records using secure electronic health record (EHR) systems, ensuring compliance with all regulatory standards.
• Collaborate with an interdisciplinary team of healthcare professionals, including psychiatrists and psychologists, through regular virtual meetings to discuss patient progress and optimize care.
• Participate in ongoing case consultations and peer review sessions, utilizing video conferencing tools to enhance collaborative care.
• Educate patients and their families on mental health issues related to end-of-life care, available treatment options, and community resources, employing digital communication effectively.

Your primary focus will be on a diverse population of patients receiving hospice care. This includes individuals facing life-limiting illnesses and their families, who require support in managing anxiety, grief, and loss. • A Master's degree in Social Work from an accredited institution is required.
• A valid license to practice as a Licensed Clinical Social Worker (LCSW) in the state is mandatory.
• A minimum of 2 years of clinical experience in providing psychotherapy, with a focus on hospice or palliative care preferred.
• Demonstrated expertise in solution-focused therapeutic interventions, with an ability to adapt these techniques for remote client interactions.
