This position allows for remote work, providing flexibility while you engage in meaningful social work practice within our Hospice Center. The role requires utilization of telehealth platforms for virtual support and assessments, ensuring that even from a distance, you maintain strong connections with patients and their families. You will collaborate with multidisciplinary teams composed of nurses, physicians, and therapists to ensure holistic care for all our patients.

As a Licensed Clinical Social Worker, your key responsibilities will include:

• Conducting thorough evaluations and assessments of patients and families facing critical care decisions.
• Participating actively in the development and implementation of individualized service plans, ensuring they reflect the unique needs and preferences of patients and their families.
• Monitoring outcomes and adjusting care strategies as part of a collaborative team, contributing to ongoing improvements in patient care and experience.
• Providing emotional and practical support to families, guiding them through end-of-life planning and bereavement processes.
• Administering evidence-based interventions designed to improve coping strategies and emotional healing.
• Developing rapport with patients and family members, creating a safe and trusting atmosphere for open dialogue and support.
• Staying compliant with all federal and state regulations governing social work and hospice care practices.
• Ensuring the safety and well-being of all individuals served by upholding ethical standards and guidelines in your practice.

Your expertise will focus primarily on patients experiencing terminal illnesses and their families, providing vital emotional support, crisis intervention, and resource navigation. The role requires sensitivity to the profound experiences of families managing end-of-life care, ensuring they have access to the emotional and practical resources necessary through a challenging time in their lives.

The successful candidate will possess the following qualifications:

• A Bachelor’s degree in Social Work is required; a Master's degree is preferred.
• Current licensure as a Clinical Social Worker (LCSW) issued by the Department of Public Health.
• A valid driver’s license for possible travel to patient locations if required.
• Demonstrated ability and confidence in working with diverse populations and adapting approaches according to individual needs.
• Excellent organization and time management skills, with the ability to manage multiple cases and deadlines effectively.
• Strong interpersonal and communication skills to engage effectively with patients, families, and the interdisciplinary team.

While not mandatory, the following experience would be advantageous:

• A minimum of one year of experience in hospice care or similar settings, focusing on end-of-life issues.
