• Act as a vital liaison between clients, their families, and outpatient service providers to ensure continuity of care.
• Engage with insurance companies for pre-certification and support utilization management processes.
• Participate actively in discharge planning and aftercare services to facilitate clients’ transition back to their communities.
• Provide therapeutic meal support to clients while modeling healthy relationships with food to promote holistic wellness.
• Deliver empathetic and affirming care to clients, taking into account their unique circumstances and intersecting identities.

Your work will primarily involve supporting individuals from various backgrounds, including those facing emotional distress, trauma, and mental health disorders. We value a culturally sensitive approach in delivering care, ensuring that all clients feel seen and heard. Understanding the diverse needs of our client population allows us to tailor our interventions to improve each client’s health outcomes significantly.

• Master's degree in Social Work from an accredited institution.
• A valid LCSW license in the state of Texas is required.
• Experience working with older adults or other specific populations preferred.
• Strong clinical and diagnostic assessment skills are essential.
• Excellent communication and interpersonal skills to foster supportive client relationships.
• The ability to work independently while also functioning effectively within a collaborative team environment.

• Experience using electronic health record (EHR) systems to manage client data and treatment plans.
• Familiarity with evidence-based therapeutic modalities and crisis intervention techniques.
• Previous experience in community-based settings or clinics is a plus.

The salary for this position ranges from $90,000 to $105,000 annually, commensurate with experience and qualifications.
