Facility: Holy Cross Hospital
Location: 1500 Forest Glen Rd., Silver Spring, MD, 20910
Shift: 8:00 AM – 4:30 PM (M–F, EOW Required)
Pay Rate: $55/hr (W2) | $60/hr (1099)
Position Overview:
Holy Cross Hospital in Silver Spring, MD is seeking an experienced Licensed Clinical Social Worker (LCSW) to join the Care Management team for a 13-week contract assignment. The ideal candidate will bring recent acute care hospital experience, with a focus on discharge planning and care coordination.
Requirements:
  • License: Active Maryland LCSW required to interview
  • Experience: Recent experience in acute care discharge planning (hospital setting)
Job Responsibilities:
  • Collaborate with interdisciplinary teams to facilitate timely and effective discharge plans
  • Perform comprehensive psychosocial assessments
  • Provide counseling and support to patients and families
  • Coordinate community and post-acute resources
