Work Environment

Key Responsibilities

• Serve as the primary liaison with families and outpatient providers, effectively communicating updates and ensuring continuity of care.
• Engage with insurance companies to navigate pre-certification and utilization management processes, advocating for the necessary resources for clients.
• Participate in discharge and aftercare planning, ensuring that clients have a comprehensive plan in place for their ongoing support.
• Offer therapeutic meal support and guidance, while modeling and fostering healthy relationships with food.
• Provide safe, empathetic, and affirming care tailored to each client’s unique circumstances, respecting their intersecting identities and individual experiences.

Patient Population / Client Focus

The Licensed Clinical Social Worker will primarily serve a diverse array of clients, including older adults experiencing various mental health challenges. Our clientele comes from different backgrounds and possesses a range of intersecting identities, requiring a culturally competent and individualized approach to care. Qualifications

• Master's degree in Social Work from an accredited institution.
• Current and valid LCSW license in the state of Virginia.

Compensation and Benefits
