Licensed Clinical Social Worker (LCSW)

Connect Health + Wellness is seeking candidates for a Licensed Clinical Social Worker, (LCSW) for Full Time and/or Part Time employment with the opportunity to work out of Connect Health + Wellness ? Bassett or Ridgeway.

Connect Health + Wellness is a Federally Qualified Health Center (FQHC) dedicated to providing accessible, high-quality medical, dental, and integrated behavioral health services to underserved and uninsured communities in the Martinsville-Henry County area. Our team also delivers chronic disease self-management education, medication assistance, and care coordination to improve health outcomes and quality of life for our patients.

Position Summary

We are seeking a compassionate and experienced Licensed Clinical Social Worker (LCSW) to join our integrated care team. This role may be performed onsite at one of our clinics or remotely, with flexible scheduling options. The LCSW provides patient-centered, evidence-based mental health services in a collaborative, team-based care model within a Patient-Centered Medical Home (PCMH) framework.

Key Responsibilities

  • Deliver comprehensive, culturally competent mental health counseling to individuals across the lifespan.
  • Perform mental health assessments and create individualized care plans based on clinical evaluation.
  • Collaborate closely with primary care providers, behavioral health professionals, and other team members to support holistic care.
  • Maintain timely, accurate documentation in the electronic medical record (EMR).
  • Participate in quality improvement initiatives, staff development, and team meetings.
  • Maintain required patient visit metrics and appointment cycle times.
  • Provide timely responses to patient and staff communications, including EMR messages and phone calls.
  • Support organizational outreach efforts and help promote access to behavioral health services.
  • Adhere to HIPAA standards and uphold the privacy and security of patient information.
  • Perform other duties as assigned consistent with the mission of the organization.
