This position allows for a flexible remote work setting, enabling you to provide critical support from the comfort of your home. Our standard work hours are Monday to Friday, from 9:00 AM to 5:00 PM, offering a balanced schedule that promotes work-life harmony.

Key Responsibilities

As an LCSW, your role will encompass a variety of important tasks, including but not limited to:

  • Acting as a liaison between clients, families, and outpatient providers to ensure seamless communication and care coordination.
  • Interacting with insurance companies for pre-certification and utilization management to facilitate timely service provision.
  • Participating in discharge and aftercare planning to support clients in their transition from intensive support to community-based resources.
  • Providing therapeutic meal support to clients, demonstrating and modeling healthy relationships with food.
  • Delivering safe, empathetic, and affirming care that respects clients' diverse identities and unique circumstances.
Patient Population / Client Focus

Qualifications

The successful candidate will possess the following qualifications:

  • A Master's degree in Social Work from an accredited institution.
  • Valid LCSW license in the state of Texas.
  • Demonstrable experience working with older adults in a clinical setting.
  • Strong clinical assessment, diagnostic, and treatment skills.
  • Excellent communication and interpersonal skills, with the ability to build rapport quickly with clients and families.
  • A proven ability to work autonomously as well as collaboratively within a multidisciplinary team.
Preferred Experience
