Work Environment

This position is primarily remote, giving you the flexibility to work from a home office while collaborating closely with our multidisciplinary team. Our work hours are Monday through Friday, from 9:00 AM to 5:00 PM, offering a predictable schedule that allows for personal commitments. Key Responsibilities

• Act as a liaison between families and outpatient providers, facilitating communication and promoting cohesive care strategies.
• Engage with various insurance companies for pre-certification and utilization management processes, ensuring that clients receive timely and appropriate access to services.
• Participate actively in discharge and aftercare planning, offering comprehensive support to patients transitioning from inpatient settings to community-based care.
• Provide therapeutic meal support, guiding clients in developing a healthier relationship with food and promoting overall wellness.
• Deliver empathetic, affirming, and non-judgmental care tailored to the unique circumstances and intersecting identities of each client.

Patient Population / Client Focus

Qualifications

• A Master’s degree in Social Work from an accredited institution is required.

Compensation and Benefits
