Job Summary

Unity Health and Rehabilitation Center is seeking compassionate and dedicated LPNs to join our part-time or per diem team. As a valued member of our staff, you will provide top-notch medical care to our residents, helping them to feel at home. Our center is committed to delivering exceptional rehabilitation and skilled nursing care, and we are looking for a skilled professional like you to help us achieve this goal.

Responsibilities

  • Provide high-quality patient care, including administering medications, wound care, and other treatments as prescribed by physicians

  • Develop and implement individualized care plans, working closely with the interdisciplinary team to ensure comprehensive care

  • Monitor patient progress, reporting changes and concerns to the healthcare team

  • Maintains accurate and detailed records of patient care and treatments

  • Participates in team meetings and contributes to quality improvement initiatives

  • Collaborates with other departments, including therapy and rehabilitation staff, to ensure a holistic approach to patient care
