Job description

  • Provides safe efficient age specific nursing care to patients/families using the nursing process in order to assist them to attain, maintain, regain, and sustain optimal health status.
  • Functions in accordance with the professional standards of the Ohio board of nursing.
  • Completes/submits required documentation.
  • Delivers effective relationship based nursing care.
  • Demonstrates technical skills in care delivery that result in safe practice and positive care outcomes.
  • Demonstrates clear communication and effective collaboration to optimize patient care and clinical operations.
  • Develops self and contributes to the professional practice of nursing.
  • Displays leadership that influences optimal clinical and operational outcomes.
  • Performs other duties as assigned.
  • Complies with all policies and standards.
  • For specific duties and responsibilities, refer to documentation provided by the department during orientation.
  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients.
  • Annual training, the UH Code of Conduct and UH policies and procedures are in place to address appropriate use of PHI in the workplace.
  • Full time 40 hours per week.
  • Monday-Friday with rotationg weekends and holidays.
  • Covering Geauga, Portage and Summit Counties and surrounding areas.
  • 100% travel to the patients homes for care.
  • 30 visits per week.
  • Adult population.

Qualifications:
  • Other Accredited Program: Completion of accredited practical nurse program or currently enrolled in a nursing program required.
  • 2+ years Nursing Experience (Level II only).
  • Knowledge of LPN scope of practice according to the Ohio board of nursing.
  • Uses a wide variety of technical equipment to meet patient needs and prescribed clinical therapies (i.e. stethoscopes, thermometers, sphygmomanometers and electronic monitoring devices).
  • Level II Only: 18 CNE's per year. • 3 CNE's in clinical specialty • At least 5 CNE's in Leadership • Other CNE requirements will be agreed upon with manager
  • Level II Only: • 3 Peer Reviews. • Committee Involvement. • 5 criteria with evidence of each.
  • Licensed Practical Nurse (LPN), Ohio and/or Multi State Compact License Required Upon Hire.
  • Basic Life Support (BLS) Required Upon Hire.
