True Motion Physical Therapy is seeking a dedicated and compassionate Home Care Physical Therapist (PT) to join our team in Union, New Jersey. This full-time position involves providing high-quality rehabilitative care to patients in the comfort of their homes, enabling them to regain mobility and improve their quality of life after injury, surgery, or illness. This position is primarily onsite, working directly with patients in their home settings. • Conduct comprehensive evaluations of patients in their home environment, assessing their physical condition and functional abilities.
• Develop and implement individualized treatment plans utilizing techniques such as therapeutic exercises, manual therapy, stretches, and modalities to alleviate pain and promote mobility.
• Continuously track and document patient progress, establishing benchmarks for rehabilitation goals and making adjustments to treatment plans as necessary.
• Foster effective communication and collaboration with patients, caregivers, and families, providing education on the rehabilitation process and the use of assistive devices when needed.
• Engage in ongoing professional development through workshops, seminars, and networking within professional associations to enhance your knowledge and skills in physical therapy.

At True Motion Physical Therapy, you will work with a diverse patient population that includes children, adults, and seniors recovering from various conditions—ranging from post-surgical rehabilitation to chronic pain management. Your role will involve tailoring treatments to meet the unique needs of each patient, ensuring that they receive personalized care and attention. By delivering therapy in a home care setting, you directly influence their ability to perform daily activities and improve their health outcomes.

• Mandatory Doctorate degree in Physical Therapy (DPT) from an accredited program.
• A valid state license in Physical Therapy is required.
• Strong written and verbal communication skills are essential to effectively communicate with patients and collaborate with healthcare team members.
• Proficiency in word processing and basic computer skills are necessary for documentation and communication purposes.
• Flexibility and agility in adjusting treatments based on patient needs and progress.

• Ideally, candidates should have at least two years of relevant hands-on experience in the field of physical therapy; however, new graduates are also encouraged to apply.

The salary for this role ranges between USD 100,000 and 120,000 annually, commensurate with experience. We offer a comprehensive benefits package that includes:
