Position Overview

We are seeking an experienced Registered Nurse (RN) to join our growing team serving the Treasure Coast area. The ideal candidate will have psychiatric nursing experience and a passion for delivering quality home health care. Youโ€™ll be responsible for providing skilled nursing care, coordinating treatment plans, and supporting patients and families in managing their health at home.

Responsibilities
  • Provide skilled nursing care to patients in accordance with physician orders and the established plan of care
  • Conduct comprehensive assessments and develop individualized care plans
  • Administer medications and treatments as prescribed
  • Monitor patient progress and communicate updates to the care team and physician
  • Educate patients and families on disease management, medication, and home safety
  • Document all patient interactions in compliance with Medicare and agency standards
  • Collaborate with interdisciplinary team members to ensure high-quality outcomes
Qualifications
  • Active Florida RN license (required)
  • 1+ year of home health and psychiatric nursing experience
  • Current BLS certification
  • Strong clinical judgment, time management, and communication skills
  • Reliable transportation and willingness to travel within the Indian River territory
