Work Environment

This position allows for a flexible work setting, primarily conducted remotely, with the unique context of providing services in a hospice center. You will have the chance to work with a diverse client population who often face complex emotional and psychological issues associated with end-of-life care. This setting requires sensitivity and a deep understanding of grief, loss, and the accompanying mental health challenges.

Key Responsibilities

• Conduct comprehensive mental health assessments, including psychosocial evaluations and risk assessments, to understand the specific needs of each client.
• Collaborate with clients to formulate individualized treatment plans that are culturally sensitive and aligned with personal goals.
• Implement evidence-based therapeutic modalities, such as cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and solution-focused brief therapy (SFBT), across individual, couple, family, and group settings.
• Provide immediate crisis intervention services and establish strong communication with other healthcare professionals to ensure continuity of care for clients in acute distress.
• Maintain accurate and timely clinical documentation, ensuring compliance with all regulatory guidelines as well as clinic policies.
• Educate clients and their families on mental health conditions and coping strategies, fostering empowerment and resilience.
• Collaborate closely with a multidisciplinary team, including psychiatrists, psychologists, nurses, and case managers, to provide holistic care tailored to each client's needs.
• Regularly evaluate treatment effectiveness through outcome assessments and adjust treatment plans as necessary to ensure optimal client progress.
• Stay current with ongoing research and best practices in the field of mental health, integrating this knowledge into daily clinical practice.
• Uphold ethical standards by maintaining confidentiality and professionalism in all client interactions.

Qualifications

• A Master’s degree in Social Work from an accredited program is mandatory for this role.
• An active and unrestricted State license as a Licensed Clinical Social Worker (LCSW) is required.
• Demonstrable ability to work both independently and as part of a collaborative, multidisciplinary team.
• Strong interpersonal and communication skills, particularly in establishing rapport with clients from various cultural backgrounds.

Preferred Experience

• Prior experience in hospice care or working with terminally ill patients is highly desirable.

Compensation and Benefits
