Job description

WelbeHealth PACE helps seniors stay in their homes and communities by providing comprehensive medical care and community-based services. We serve the most vulnerable seniors with better quality and compassion in a value-based model.

Our Home Care Assistants are a vital part of our team, providing aid to our participants in their homes as well as contributing to the care planning process as a member of our interdisciplinary team (IDT). Reporting to the Home Care Supervisor, the Home Care Assistant will provide safe delivery of personal care services to WelbeHealth participants in their home, as scheduled and directed.

This role is different because Home Care Assistants at WelbeHealth: 

  • Work a stable, full-time schedule - unlike traditional home care roles, you will be assigned participants to fill a full workday, and will be re-assigned in cases of cancellations or unforeseen circumstances
  • Visit 2-5 patients per day depending on participant needs and care plans, working shorter individual shifts that still add up to an 8-hour workday
  • Receive reimbursement for travel time and mileage between participants

On the day-to-day, you will: 

  • Provide hands-on personal care and assistance with activities of daily living (ADLs) to participants in their homes, including but not limited to bathing, dressing, grooming, dressing, toileting, and physical lifting when transferring participants
  • Assist participants with Instrumental ADLs, including but not limited to laundry, light housekeeping, shopping, and light meal preparation
  • Measure and document participants' vital signs as instructed
  • Act as a medical escort on vehicles to medical appointments, supporting participants with safe transfer in and out of their homes
  • Assist the Rehabilitation team by carrying out home exercise programs as indicated in the care plan developed by the IDT, after sufficient training as needed
  • Provide translation services as needed

Job requirements include: 

  • High school diploma or equivalency
  • Current Basic Life Support (BLS)/First Aid Certification required
  • Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification preferred
  • Bilingual preferred
  • Must be registered with Home Care Registry prior to start date
  • Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred
