Centurion Health is proud to be the provider of comprehensive healthcare services Indiana Department of Correction.
We are seeking a Transitional Healthcare Liaison for a full-time position at South Bend Parole District 8 in South Bend, Indiana.
The Transitional Healthcare Liaison will be responsible for engaging with recent parolees post-release to assist with and support the continuum of care and provide resources and community-based services. The Transitional Healthcare Liaison will provide education and support to the released offenders to enable a successful transition into their community, connect them to community resources, and assist in obtaining services.
  • Bachelor's degree in Psychology, Social Work, Counseling or related field from an accredited institution
  • Two years of experience working in a correctional setting
  • Experience with/knowledge of community and state programs and services
  • Two years of experience in re-entry services or release planning preferred
  • Must maintain current tuberculosis screening documentation and active CPR and Automated External Defibrillator (AED) use certification
  • Must be able to pass background investigation and obtain agency security clearance where applicable

Available Shift: full-time days, Monday-Friday
