Job description

Join a group of passionate advocates on our mission to improve the lives of youth! Rite of Passage Team is hiring for a Transitional Care Coordinator at Southern California Treatment Program in Torrance, CA

Southern California Treatment Programs (SCTP) has been serving youth from Child Welfare, Social Services, and probation systems around Los Angeles and Orange County since 2012. Our nine CARF-accredited group homes, all of which are licensed Short-Term Residential Therapeutic Programs, provide a safe and nurturing environment in which the youth can learn, grow, and develop strong ties with their local community. SCTP utilizes a community-centered approach, enabling our youth to attend school, hold a job, volunteer, and engage in community projects.

Pay: Rate starting at $23.00 per hour.

Perks and Benefits: You will be eligible for Medical insurance on the first of the month following 60 days of full-time employment. What you will do: You will be responsible for administrating, developing, and facilitating the completion of case management services in accordance with the needs of each assigned resident. You will act as liaison between our residents and all site departments, parents/legal guardians, and placing agency officials, and ensure successful transition back to the community.

To be considered you must: Demonstrate empathy, patience, and respect, along with a genuine desire to work with troubled teens. ~ Successfully pass a criminal background check, drug screening, physical exam, and TB test. ~ Hold a current State Driver’s License with an acceptable driving record for the past three years. ~ Retain an Associates level degree in a related field with at least one year of experience working with at-risk juveniles and their families or other related experience; Bachelor’s degree preferred. Relevant experience includes, but not limiting to working with youth in mental health setting, treatment services, residential, schools, after-school programs, or coaching sports.

Shifts: Monday – Friday 9:00 am - 6:00 pm
