What is a transition pharmacy?

A Transition Pharmacy is a specialized pharmacy service that helps patients manage their medications when moving between different care settings, such as from hospital to home or to a rehabilitation facility. The goal is to ensure patients have access to the right medications, reduce the risk of medication errors, and improve health outcomes during these transitions. Transition Pharmacy teams often provide medication counseling, coordinate with healthcare providers, and help with medication reconciliation to prevent gaps in care. These services are particularly important for patients with complex medication regimens or chronic conditions.
