Position: Transition Coordinator-CHSD 218
Starting Date: Anticipated August 10, 2026Qualifications: Hold a registered, valid Illinois Professional Educators License with Endorsement in one or more of the following: LBS I, LBS II, School Social Worker, School Psychologist, School Counselor, Speech-Language Pathologist, or other related service area. Demonstrated experience in secondary transition planning for students with disabilities; Experience collaborating with families, school personnel, and community/adult service agencies; Knowledge of state and federal special education regulations related to transition services .
Performance Responsibilities: Transition Planning and Student Support- Provide consultation and support to school teams in developing and implementing compliant transition plans within IEPs.
- Assist in developing measurable postsecondary goals aligned with student strengths, interests, preferences, and assessment data.
- Support creation and review of individualized courses of study aligned to postsecondary outcomes.
- Facilitate and interpret age-appropriate transition assessments.
- Promote and support meaningful student participation in IEP and transition meetings.
- Assist in completing the Summary of Performance during the student's final year of eligibility.
- Support instruction and planning for daily living and independent living skills, including self-care, transportation, money management, healthcare access, and community participation.
- Provide coaching, observation, and feedback to classroom teams to support fidelity of transition plans.
Collaboration With Families and Agencies- Maintain ongoing communication with families to support long-term transition planning.
- Assist families in understanding adult service systems, eligibility requirements, and timelines.
- Serve as a liaison with community, county, state, and federal agencies, including the Division of Rehabilitation Services (DRS).
- Identify and coordinate community resources related to employment, training, postsecondary education, and adult services.
- Support and identify parent training opportunities to enhance family engagement.
Division of Rehabilitation Services (DRS) Coordination Duties- Prepare and submit required documentation to DRS, including updated IEPs.
- Create and distribute DRS enrollment packets (IEPs, evaluations, transcripts, and required student information).
- Track signed Individualized Plans for Employment (IPEs) and Plan Agreements.
- Maintain accurate student case files and electronic documentation.
- Enter and update transition services, job training hours, employment data, and worksite information in required systems.
- Complete case notes and documentation based on student needs and services provided.
- Monitor and document completion of knowledge checks and MATCH activities for enrolled students.
- Coordinate with vocational coordinators regarding referrals, enrollment status, and service delivery.
Compliance and Data Management- Ensure all transition components of IEPs meet IDEA and Illinois requirements.
- Monitor timelines and compliance related to transition services.
- Maintain accurate records of student participation in work-based learning and community employment.
- Support district and Cooperative reporting requirements related to transition outcomes.
Salary Information: Certified Salary Range Per CBA: $61,406 - $83,386 (+ longevity)-depends on experience
