Job description

Job Title: Care Transitions Coordinator - RN/LPN - Weekends

Job Location: Brentwood, TN

Job Type: Full-Time

Position Overview

The Care Transition Coordinatorโ€™s primary responsibility is to facilitate a seamless transition for patients discharged from a facility setting to home health care. The role coordinates patient admissions, assesses and plans options and services, and ensures all documentation is complete to support timely transitions to home health care.

Responsibilities
  • Assist patients in navigating postโ€‘discharge and transition needs
  • Meet admissions and account goals and maintain accurate CRM records
  • Attend community events and respond promptly to patient and provider concerns
  • Report progress on growth strategies
  • Assist the agency with preparing for and accepting patients
  • Assist the Administrator with execution of contracts for facilityโ€‘based services for home health patients
  • Explain home health services and agency procedures to patients and their families
  • Assess postโ€‘discharge educational coaching needs
Qualifications
  • One (1) year of home health experience
  • Current RN or LPN license in Tennessee
  • Thorough understanding of home health qualifying criteria and coverage guidelines
  • Current driverโ€™s license, vehicle insurance, and access to a dependable vehicle or public transportation
  • Reliable ability to commute to multiple locations daily within the assigned region
