Responsibilities:

  • Maintains billings expectations and a caseload established with the designated supervisor and determined by the composition and intensity of caseload, geographical area, and specific service structure in the region on a monthly, quarterly and yearly basis.
  • Provides support in the development of children's physical skills, cognitive/academic skills, self-help skills, language/communication skills, social/emotional/coping skills in a least-restrictive environment.
  • Utilize a wrap-around approach to assist with assessment, problem solving, strength identification, parent education, facilitating social skills groups, utilizing a wrap-around approach
  • Assesses client, family, and environment to determine the appropriateness and need for services.
  • Co-creates a comprehensive family plan with family, identifying strengths, family needs, and interests, setting short- and long-term goals and tracking progress. 
  • Utilizes a strengths-based approach to address child and family challenges.
  • Conducts home visits for all families to identify areas of need and monitor progress to treatment plan.
  • Provides service coordination linking families to wraparound resources, referrals, and community partners.
  • Participates in and coordinates team meetings of families.
  • Assists the person's entry into service and monitors the needed services throughout the service system.
  • Follows State of Montana and AWARE rules, policies, procedures, and guidelines.
  • Prepares reports, applications, and supporting documentation as requested by the supervisor within timelines established by that supervisor.
  • Performs various case management activities, such as attending meetings, arranging for individual therapy, locating additional supports/resources, etc.
  • Acts as child and family advocate representing family's needs, to agencies and community service providers.
  • Understands child development, best practices for children with disabilities, and ability to maintain confidentiality.
  • Utilizes H.E.L.P. (including therapeutic holds) when it is determined that a child is a danger to self or others; also performs basic First Aid and/or CPR when needed.
  • Attends and participates in mandatory training, in-services, staff meetings, and Corporate Committees as assigned.
  • Participate in community engagement activities including meetings, events, inter-agency collaboration to promote program awareness.
  • Required to pass and maintain required AWARE training and/or certifications as defined in the FACT training plan
  • Travels within the geographical area of the assigned caseload to provide needed services.
  • Must demonstrate - on a consistent basis - the willingness and ability to productively utilize work time to meet the needs of the individuals being served as directed, with the understanding that specific fiscal goals (billing, case load number maintenance, weekly) must be met monthly.
  • Records notes, billing units, assessments, referrals, and other data entry assignments via AWARE's Electronic Healthcare Record System
  • Perform other duties as assigned.

Talents, skills, and abilities:

  • Ability to assess and to respond to the needs of children and their families. Requires a knowledge of child development, case management techniques, community resources, and knowledge of how to access services. Knowledge of a strength model perspective and an understanding of the family systems approach. Ability to establish strong working relationships with adults and children. Ability to work flexible hours, including evenings and weekends.
  • Ability to synthesize available resources and integrate strategies into an effective plan that appropriately addresses the challenges presented by each client. Strong oral and written communication, organizational and problem solving abilities. 
  • A BS/BA Degree in a Human Services Field and one (1) year of direct work experience. An equivalency may be considered. A Valid Montana Driver's License, along with a clean motor vehicle record required.
  • Must have, or be willing to obtain, a valid Montana Driver's License. Must have an acceptable driving record; as well as an acceptable criminal and protective services background check.
