Job description

Hinds LifeCare, one of the most well-known and well-respected nonprofit agencies in California's Central Valley and a pioneer in the practice of hospice and palliative care, is seeking a Hospice Inpatient Hospital Liaison to join our team.
This position reports directly to the Director of Hospice Access and Inpatient Services. When serving in the Liaison capacity, the RN conducts clinical evaluations in collaboration with hospitals, facilities, and other healthcare partners to determine hospice eligibility and complete admissions for both inpatient and outpatient services, under the direct supervision of the Clinical Administrator. This role works closely with the Patient Access team and the Business Development/Outreach team to strengthen relationships with referral sources across Fresno, Madera, and Merced counties and to support seamless transitions into hospice services.
The Liaison represents Hinds LifeCare within hospitals and facilities by conducting clinical evaluations, coordinating admissions, and nurturing referral relationships.
  • Respond promptly to requests for clinical evaluations and informational meetings from referral sources, patients, and families.
  • Perform hospice admissions for both inpatient and outpatient services.
  • Explain hospice philosophy, services, and levels of care (including GIP) with expertise and clarity.
  • Assess patient and family needs and goals to ensure appropriate hospice placement.
  • Coordinate with team members and the Medical Director to expedite admission decisions.
  • Complete informed consent with patients and families accurately and compassionately.

  • Provide high-quality customer service and communication to strengthen referral relationships.
  • Collaborate with the Director of Inpatient Hospice Services to support and maintain census goals.

Qualifications include but are not limited to:
  • Completion of a professional nursing program accredited by the Board of Registered Nursing with completion of a baccalaureate program in nursing required. Masters Degree preferred.
  • Two (2) years of leadership experience in a hospice environment preferred.
  • Certification in Hospice and Palliative Nursing, preferred.
  • Demonstrated ability to work with health team personnel with three (3) years of nursing experience in a hospice/palliative care setting.
  • Possess ability to function independently with strong clinical skills and critical thinking abilities as relates to patient care and staff responsibilities.
  • Exhibits sensitivity to cultural differences and have substantive knowledge of current trends in home health care, federal and state regulation, state licensing requirements and other standards related to home health care
  • Current RN Licensure, CPR, recent physical and TB, Tdap, MMR, Varicella, HepB immunizations.
  • Must have a valid drivers license and a willingness to use personal vehicle in the course of employment.
  • Must show proof of automobile insurance, proof of current auto registration and acceptable driving record (no more than 3 points within a 36-month period).
  • Ability to meet the physical demands of the job (i.e., Push, Pull, Lift/Carry 35 lbs.)
