Medical Case Manager

Department: Health Home

Position Summary

Arundel Lodge is seeking a dedicated and organized Medical Case Manager to support the coordination and delivery of high-quality healthcare services for individuals enrolled in our Health Home program. The Medical Care Manager serves as a key liaison between consumers, healthcare providers, pharmacies, hospitals, and community resources to ensure continuity of care and positive health outcomes.

The ideal candidate will possess strong clinical coordination skills, knowledge of medical terminology and healthcare systems, and a commitment to recovery-oriented care.

Key Responsibilities :Clinical Care Coordination
  • Schedule and manage laboratory testing, including Clozapine monitoring and medication level testing.
  • Monitor and manage toxicology screenings, including random and scheduled testing.
  • Obtain and document vital signs.
  • Assist prescribers and nursing staff with prior authorization requests.
  • Advocate for individuals served regarding psychiatric, medical, and medication-related needs.
  • Coordinate care with physicians, hospitals, pharmacies, nursing facilities, and specialists.
  • Assist consumers in locating providers who accept their Managed Care Organization (MCO) or Medicare Advantage plans.
  • Manage Clozapine services and serve as REMS designee.
  • Act as Medication Agent for the Medical Director and other providers.
  • Conduct medication box and Medication Management Record (MMR) audits.
  • Perform medication reconciliation and assist with medication refill processes when necessary.
  • Clarify medical and psychiatric provider orders as needed.
Program & Administrative Support
  • Assist the Health Home Director with assigned projects and operational tasks.
  • Manage scheduling and communication with Anne Arundel County Health Center (AACHC) providers and staff.
  • Complete Health Home documentation and clinical notes as required.
  • Support Primary Care Clinic operations, including vital signs collection, appointment coordination, and provider support.
  • Provide consultation to providers during the absence of the Health Home Director.
  • Assist with psychiatric and medical hospital admissions, discharges, and transitions of care.
  • Maintain accurate documentation, including treatment summaries, contact notes, and health records.
  • Manage office supplies, equipment, and administrative resources.
  • Monitor and manage care alerts.
  • Participate in on-call responsibilities.
  • Perform additional duties within the scope of the Medical Care Manager role as assigned by nursing staff, physicians, nurse practitioners, or agency leadership.
$30 - $32 an hour
Qualifications Education
  • Bachelor's Degree (BS/BA) or Associate Degree (AA) in Health Administration, Human Services, Nursing, or a related field preferred.
  • Current Registered Nurse (RN) license in the State of Maryland preferred.
Experience
  • Minimum of one (1) year of experience in the Human Services, Behavioral Health, Healthcare, or related field preferred.
Knowledge, Skills, and Abilities
  • Knowledge of medical terminology, healthcare practices, and clinical protocols.
  • Ability to provide education and support to consumers and staff.
  • Strong care coordination and advocacy skills.
  • Ability to collaborate effectively with multidisciplinary healthcare teams.
  • Strong organizational, documentation, and time-management skills.
  • Proficiency in Microsoft Office and electronic health record systems.
  • Ability to work independently with minimal supervision.
  • Ability to prioritize and complete tasks in a fast-paced environment with frequent interruptions.
