Description:

Access TLC is currently seeking a HCBA Registered Nurse (RN) to join our team in the HCBA department. The Home and Community Based Alternatives(HCBA) Waiver provides care management services to persons at risk for nursing home or institutional placement. The care management services are provided by a multidisciplinary care team comprised of a nurse and social worker. The care management team coordinates Waiver and State Plan services (e.g., medical, behavioral health, In-Home Supportive Services, etc.), and arranges for other available long-term services and supports available in the local community.

The HCBA Registered Nurse (RN) is responsible for assessing, care planning, assisting with locating, coordinating, and monitoring services for community-based participants on the waiver. In- person visits will be in the Los Angeles areas.

This position is full time. Initial assessment/reassessment visits are paid $150/visit and Level 3 Follow Up Visits/Plan of Treatment visits are paid at $50/visit. Mileage is also reimbursed.

Requirements:


1. Registered nurse with current licensure to practice professional nursing in the State of California.

2. Minimum of one year nursing experience.

3. Demonstrates excellent observation, verbal and written communication skills.

4. Possess and maintains current CPR certification.

5. Valid California Driver's License with proof of a good Motor Vehicle Record.

6. Current proof of auto insurance with state minimum (or greater) coverage.
