Licensed Practical Nurse – LTC

TLC Nursing Associates, Inc. is seeking a compassionate and reliable Licensed Practical Nurse (LPN) – Long-Term Care (LTC) to provide direct nursing care and support to residents in a long-term care facility. This role is essential in ensuring high-quality, continuous care for individuals with chronic health conditions or functional limitations.

Job Responsibilities
  • Deliver bedside nursing care including administering medications, treatments, and wound care under the supervision of an RN or physician.
  • Assist with admission assessments, care planning, and discharge processes for residents.
  • Monitor residents' health status and report any changes to the supervising nurse or healthcare provider.
  • Support residents with activities of daily living (ADLs) and maintain a safe, respectful care environment.
