) LPN RN Charge Nurse Per Diem 7 AM to 3 PM or 3 PM to 11 PM
Manchester Rehabilitation and Healthcare Center is seeking dependable and clinically strong LPN or RN Charge Nurses for per diem day and evening shifts.
The Charge Nurse provides direct resident care while supervising CNAs and other nursing personnel. Candidates must demonstrate strong clinical judgment, leadership, organization, and the ability to manage a nursing unit confidently.
Available Shifts
  • 7 AM to 3 PM
  • 3 PM to 11 PM
  • Per diem schedule
  • Weekend and holiday availability may be required

Responsibilities
  • Provide direct nursing care according to resident care plans and provider orders
  • Direct, assign, and supervise CNAs and other nursing personnel
  • Monitor residents and respond promptly to changes in condition
  • Administer medications and treatments accurately
  • Complete timely clinical documentation and required reports
  • Coordinate admissions, transfers, and discharges
  • Communicate with residents, families, providers, and nursing leadership
  • Educate, coach, counsel, and redirect staff as needed
  • Address employee performance and policy-compliance concerns
  • Complete accident and incident reports
  • Support discharge planning and interdisciplinary care coordination
  • Participate in quality assurance and survey-readiness activities
  • Maintain compliance with federal, state, and facility requirements

Qualifications
  • Current, active, and unencumbered Connecticut RN or LPN license
  • Graduate of an approved RN or LPN nursing program
  • Strong nursing assessment and decision-making skills
  • Leadership and supervisory ability
  • Ability to make sound independent decisions
  • Knowledge of long-term care nursing practices and regulations
  • Strong communication, organizational, and interpersonal skills
  • Comfortable using computers and electronic medical records
  • Long-term care, rehabilitation, or charge nurse experience preferred
