Chronic Care Management Coordinator - RN

Chronic Care Management Coordinator - Registered Nurse

Sweetwater Hospital Association is seeking a Chronic Care Management Coordinator to join our healthcare team. The Chronic Care Management Coordinator is a Registered Nurse responsible for coordinating comprehensive chronic disease management services for patients enrolled in the Chronic Care Management Program. The position provides proactive care coordination, patient education, care transition management, and collaboration with providers, specialists, rehabilitation staff, community partners, and caregivers to improve patient outcomes, reduce avoidable healthcare utilization, and support long-term disease self-management. This position is also responsible for program development and oversight.

Education:

  • Graduate of an accredited school of nursing with current RN license in the State of Tennessee, BSN preferred.
  • Certified Case Manager (CCM), Accredited Case Manager (ACM), or certification in Care Coordination preferred.
