What You’ll Be Doing:

You will be a welcomed member of the Long Term Care team at Name  Community, working in a fast-paced environment where you will make a meaningful difference in residents’ lives.  As a Resident & Family Experience Coordinator, you’ll be the main point of contact for residents & families at the Community.  You will build relationships with residents and their loved ones by coordinating care and services that enrich their daily health and well being.  As an individual and part of a wider team, you will be a strong contributor to the culture of the Community, where the focus is always on cultivating happiness in daily life.  

How You’ll Succeed

  1. Provide Care and Service Coordination to Resident and Families
  • Lead the resident move in, transfer, and move out process
  • Provide social service work to residents living in long term care
  • Collaborate closely with the leadership team to resolve concerns raised by residents & families promptly to ensure all parties are satisfied
  1. Promote service excellence and hospitality
  • Coordinate resident & family satisfaction surveys and collaborate with the team to build action plans based on feedback to improve resident and family experience
  • Coordinate and facilitate resident care conferences
  • Engage community guest speakers to conduct in-service sessions related to long term care and services
  1. Support the leadership team to improve resident care and services 
  • Contribute to the development of quality improvement plans to increase quality of care and reduce risk
  • Conduct assigned Quality Management activities scheduled
  • Prepare data analysis and trends on a quarterly basis related to resident/family feedback

Who You Are:

  • You are compassionate and dedicated to meaningful work that supports seniors in living life with comfort, dignity, and respect
  • You have completed a College/University degree
  • You are a provincially registered Social Services Worker
  • You have 3 years’ experience in healthcare, social services or related sector 
  • You are customer focused and detailed oriented.  You can multi-task and collaborate with leaders and team members
  • You exercise tact and diplomacy in working with seniors and their families
