Visit Nurse

Our Home Health department is looking for a temporary RN as a Visit Nurse. The Visit RN helps provide continuity of care by providing nursing visits for care of patients as delegated by the Case Manager RN. The Case Manager RN remains responsible for the overall planning and coordination of care for a caseload of patients. The Visit RN member of the interdisciplinary team provides nursing care to the patient in their place of residence as prescribed by the physician. This position must comply with applicable laws, regulations, and policies of the agency. The Visit RN keeps the Case Manager RN informed about patient progress, recommended actions, physician orders, and plans for the next visits. Individuals assigned to this role are expected to apply full working knowledge of nursing principles and practices. Demonstration of problem-solving skills and the ability to make independent decisions in complex work situations under general supervision is required. An associate's degree is required, with a BSN preferred.
