Job Title

Registered Nurse (RN) for Long-Term Care (LTC) Unit

Job Description

We are seeking experienced Registered Nurses (RNs) to join our Long-Term Care (LTC) Unit. This division includes three distinct units, offering both long-term and short-stay care as well as hospice and palliative care. As an LTC RN, you will provide compassionate, patient-centered care while coordinating with interdisciplinary teams to ensure the best outcomes for residents.

Key Responsibilities:

  • Deliver and supervise all aspects of direct patient care using the nursing process (assessment, planning, implementation, and evaluation).
  • Focus on patient transitions across the care continuum, including patient and family education.
  • Manage activities of daily living (ADLs), including monitoring hydration, blood pressure, weight, and nutrition (including tube feeding).
  • Administer medications and treatments, including IV therapy and blood transfusions, following Barcode Medication Administration (BCMA) protocols.
  • Provide care for behavioral patients and participate in interdisciplinary team discussions.
  • Handle admissions, discharges, internal/external transfers, and emergency response procedures.
  • Document care thoroughly in the electronic medical record (EMR).
  • Delegate responsibilities to ancillary staff effectively and transport patients as needed.

Work Schedule:

Rotating eight (8), ten (10), and twelve (12)-hour shifts, including days, nights, evenings, weekends, and holidays.

Requirements:

  • Current, unrestricted RN license in one of the United States, Territories, or the District of Columbia.
  • BLS (Basic Life Support) certification.
  • Minimum of three (3) years of experience in acute care or other relevant nursing areas.
  • Proficiency in managing patient ADLs, BCMA, IV therapy, and behavioral care.
  • Strong communication, organizational, and team collaboration skills.

Credentialing Requirements:

  • Completed RN application
  • Updated resume
  • Copy of BLS certification
