What we want
The Maternal Health RN Care Coordinator supports the Texas A&M Rural and Community Health Institute (RCHI) Maternal Health Program through telehealth care coordination, patient education, remote patient monitoring, and support for the maternal health program across rural Texas. This position coordinates prenatal and postpartum services, supports rural clinical partnerships, and assists with maternal safety and obstetric emergency simulation training initiatives. The RN Care Coordinator works collaboratively with APRNs, physicians, and rural healthcare teams to improve maternal health outcomes and expand access to evidence-based care in underserved communities. Responsibilities include patient education, remote-monitoring review, care coordination, simulation-training support, quality-improvement activities, and outreach initiatives. This position is primarily based at the Texas A&M-Fort Worth campus, with travel to rural partner sites as needed.

Responsibilities
Telehealth Patient Coordination & Remote Monitoring
Serves as the primary nurse coordinator for patients enrolled in the RCHI telehealth maternal health program.
Conducts patient intake, screening, and onboarding for telehealth maternal health services.
Coordinates scheduling, follow-up visits, and continuity of care between telehealth providers, rural clinics, and delivering hospitals.
Monitors remote patient data including but not limited to:
Blood pressure
Pulse and oxygenation
Temperature
Blood glucose
Weight
Fetal heart rate
Escalates abnormal findings to the APRN or supervising provider.
Reviews laboratory and diagnostic results and communicates clinically relevant findings to the appropriate APRN or physician for further evaluation and management.
Implements provider-directed care plans and standing orders in accordance with clinical protocols.
Accepts and documents verbal or telephone orders from authorized providers (APRN or physician) in accordance with organizational policies and nursing scope of practice.
Prepares and transmits prescriptions and medication refills under provider direction in accordance with established protocols and regulatory requirements.
Provides patient navigation and referral coordination for behavioral health, social services, lactation support, and specialty care.
Patient Education & Community Support
Provides evidence-based education to pregnant and postpartum patients on topics such as:
Prenatal care expectations
Hypertension and preeclampsia warning signs
Gestational diabetes monitoring
Breastfeeding support
Postpartum recovery
Mental health and postpartum depression
Develops patient education materials and virtual classes for rural populations.
Virtual CenteringPregnancy classes
Supports group prenatal education sessions or telehealth classes.
Reinforces care plans established by the APRN.
OB Emergency Simulation Training Support
Assists with the planning and implementation of rural obstetric emergency simulation trainings.
Supports simulation scenarios related to:
Postpartum hemorrhage
Shoulder dystocia
Hypertensive crisis
Maternal cardiac arrest
Emergency delivery in the ED
Assists with simulation equipment setup and logistics.
Participates in simulation facilitation and debriefing sessions.
Supports training documentation and program evaluation.
Program Development & Quality Improvement
Supports maternal health program expansion across rural partner sites.
Assists with data collection for program evaluation, grant reporting, and maternal outcome tracking.
Participates in quality improvement initiatives related to maternal safety and access to care.
Assists with workflow development for telehealth services.
Required Education and Experience
Associate's degree in Nursing.
Four years of clinical nursing experience in obstetrics, maternal-newborn nursing, labor and delivery, or maternal health programs.
Required Licenses and Certifications
Active and unrestricted RN license in the State of Texas
Basic Life Support (BLS)
Licensure/certification must bemaintainedfor continued employment.
Preferred Qualifications
Bachelor's degree in Nursing.
Experience practicing in rural or underserved settings.
Experience with telehealth visits and electronic medical records (EMR) systems.
Experience in perinatal quality improvement initiatives (e.g., maternal safety bundles,TexasAIM, QI collaboratives).
Experience in basic ultrasonography.
Experience in prenatal, childbirth, and lactation education.
International Board Certified Lactation Consultant (IBCLC), Certified Lactation Counselor (CLC), or equivalent breastfeeding education training.
Electronic Fetal Monitoring Certification (NCC or AWHONN).
Registered Nurse Certified in Inpatient Obstetrics (RNC-OB) through NCC
Neonatal Resuscitation Program (NRP)Certification
Advanced Cardiac Life Support (ACLS)Certification
Experience providing maternal patient education, care coordination, and clinical assessment for pregnant and postpartum patients.
Experience reviewing clinical data such as laboratory results, remote monitoring data, and patient-reported symptoms; recognizing abnormal maternal or fetal findings; and escalating concerns to providers for timely clinical decision-making.
Experience working collaboratively with physicians, advanced practice providers, and interdisciplinary healthcare teams.
Experience in obstetric triage, labor and delivery triage, maternal-fetal medicine clinics, or obstetric emergency department settings with demonstrated ability to recognize and escalate maternal warning signs.
Knowledge, Skills, & Abilities
Knowledge of evidence-based perinatal care practices and maternal safety standards.
Knowledge of telehealth delivery and documentation standards.
Knowledge of rural healthcare systems, multiple EMRs, and general medical billing and coding principles.
Proficiencyin virtual meeting platforms (e.g., Zoom, Microsoft Teams) and Microsoft Office Suite.
Excellent written and verbal communication skills with the ability to educate patients and collaborate with interdisciplinary teams.
Strong organizational, time management, and documentation skills.
Ability to work independently with minimal supervision whilemaintainingaccountability and clinical judgment.
