Clinical Consultant – Enhanced Care Management (ECM)

About Fathers To Founders

Fathers To Founders (FTF) is a nonprofit, community-based organization partnering with Anthem Blue Cross to deliver Enhanced Care Management services through California’s CalAIM initiative.

Our mission is to support vulnerable Medi-Cal members—particularly individuals experiencing homelessness, behavioral health challenges, and other complex needs—through coordinated, trauma-informed, whole-person care.

Position Overview

Fathers To Founders is seeking an experienced Clinical Consultant to provide clinical oversight, consultation, and guidance for our ECM program. The Clinical Consultant will support Lead Care Managers and other members of the interdisciplinary care team while helping ensure that assessments, care plans, documentation, and service delivery meet applicable clinical and program requirements.

This is a part-time, hybrid consulting position requiring approximately five hours per week initially, with the possibility of additional hours as the program develops. Most responsibilities may be completed remotely, with periodic in-person participation in team meetings, trainings, case conferences, and quality-improvement activities as needed.

The ideal candidate is available to begin immediately and prepared to quickly become actively involved in program operations.

Compensation and Schedule

  • Approximately five hours per week initially
  • $50–$60 per hour, depending on qualifications and experience
  • Primarily remote, with periodic in-person participation as needed
  • Immediate availability strongly preferred

Key Responsibilities

  • Serve as the clinical subject-matter expert for ECM service delivery, documentation, and compliance.
  • Provide clinical supervision, consultation, and guidance to Lead Care Managers, Community Health Workers, peer specialists, and other care team members.
  • Review and co-sign assessments and individualized care plans for clinical accuracy, appropriateness, completeness, and quality.
  • Participate in interdisciplinary case conferences, team huddles, and ongoing care reviews.
  • Support the care team in addressing complex behavioral health, medical, substance-use, housing, and social-service needs.
  • Coach and mentor team members in trauma-informed care, motivational interviewing, harm-reduction practices, and person-centered care.
  • Help ensure alignment with California Department of Health Care Services, CalAIM, health plan, and ECM requirements.
  • Contribute to continuous quality improvement, staff development, workflow refinement, and program development.
  • Identify clinical or compliance concerns and recommend appropriate corrective actions or escalation.
  • Remain available for clinical consultation during agreed-upon hours through Zoom, telephone, or HIPAA-compliant communication platforms.
  • Perform other clinically related duties consistent with the needs of the ECM program.

Required Qualifications

Applicants must meet one of the following qualification pathways:

  • Hold an active, unrestricted California license as a Registered Nurse, Licensed Clinical Social Worker, Licensed Marriage and Family Therapist, Licensed Professional Clinical Counselor, Psychologist, Physician, or Pharmacist; or
  • Hold an active Certified Case Manager credential with at least five years of relevant clinical experience and documented supervisory experience.

Applicants must also have:

  • A current and active National Provider Identifier (NPI) number.
  • Immediate or near-immediate availability to begin supporting the program.
  • Experience in care management, behavioral health, complex case management, or a closely related field.
  • Knowledge of social determinants of health and their effects on health outcomes.
  • Experience supervising or supporting interdisciplinary teams and non-licensed care team members.
  • Strong clinical judgment and the ability to review assessments, care plans, and case documentation.
  • Excellent organizational, written, verbal, and interpersonal communication skills.
  • Ability to maintain appropriate professional boundaries and protect confidential health information.
  • Comfort using virtual meeting platforms, electronic health record systems, and other remote-work technologies.
  • Familiarity with Medi-Cal populations or healthcare delivery-system reform.

Preferred Qualifications

  • Prior Enhanced Care Management experience is a major plus.
  • Knowledge of ECM encounter documentation and health plan reporting requirements.
  • Experience supporting individuals living with serious mental illness, substance-use disorders, housing instability, or homelessness.
  • Experience working with culturally diverse and historically underserved communities.
  • Bilingual or bicultural skills aligned with the communities served by Fathers To Founders.
