Job Summary:

We are seeking a highly skilled and compassionate Clinical Liaison to join our team at Reliant Care Management Company for the Kansas City, MO area (remote with onsite visits when needed). As a Clinical Liaison, you will play a vital role in facilitating efficient and effective communication between our healthcare facilities and healthcare providers to ensure seamless patient care. Key Responsibilities:

• Develop and maintain strong relationships with healthcare providers, facilities, and other stakeholders to ensure effective communication and coordination of patient care
• Conduct clinical interviews with patients to gather information and assess their needs, goals, and preferences
• Identify and address potential barriers to care, such as social, emotional, or environmental factors
• Collaborate with healthcare providers to develop and implement individualized care plans, ensuring seamless transitions between care settings
