LanceSoft is seeking a travel nurse RN Home Health for a travel nursing job in Portland, Oregon.

Job Description & Requirements

Responsibilities

  • Provide skilled nursing care to patients in their homes, including comprehensive assessments, medication management, wound care, and patient education.
  • Develop, implement, and update individualized care plans in collaboration with physicians and interdisciplinary teams.
  • Complete accurate and timely OASIS assessments and documentation.
  • Utilize Epic Remote Client, Rover, and OASIS for clinical documentation.

Requirements

  • Active Oregon RN License and Washington RN License (required)
  • Minimum 1 year of Skilled Home Health RN experience
  • OASIS documentation experience (required)
  • Current BLS (BCLS) certification
  • Valid driver's license, reliable personal vehicle, and auto insurance
  • COVID-19 primary vaccination series plus one booster (required)
