Job description

Join the Home Health team as a Physical Therapist! You will enjoy the flexibility, variety, and autonomy that experienced professionals deserve. Primarily serving the St. Cloud and surrounding areas, you will work with patients of almost all ages, allowing you to build meaningful connections within your region. You will typically visit the homes of four to six patients per day, with the freedom to schedule appointments at times that work best for both you and your patients. Your responsibilities will include reviewing treatment orders and medical records, performing patient evaluations, and establishing and implementing customized treatment programs. Additionally, based on your experience, you may be entrusted with supervisory duties for students, Physical Therapist Assistants, or Nursing Assistant/Home Health Aides.

Schedule:

  • Full-time | 72-80 hours every two weeks 
  • Monday-Friday 8 a.m.-5 p.m. 
  • No nights, weekends, or holidays!

Pay and Benefits:

  • Starting pay begins at $45.38 per hour and increases with experience
    • Pay range: $40.33-60.51 per hour
  • Paid milage reimbursement 
  • $5,000 hiring bonus and $5,000 relocation bonus

Qualifications:

  • Graduate from an accredited school of physical therapy.  New or upcoming graduates welcome to apply!  
  • One year of experience as a Physical Therapist providing treatment to various patient groups preferred.
  • Licensed by the MN Board of Physical Therapy as a physical therapist or current temporary permit required. License required within 90 days from date of exam.
  • Transportation available for off campus activities required. 
  • A valid Minnesota driver's license, a driving record in good standing, and the minimum amount of insurance liability coverage required.
  • Current American Heart Association (AHA) BLS Healthcare Provider card required prior to completion of unit orientation.

To be eligible for the hiring bonus you must be an external candidate who is hired into an eligible full-time or part-time benefited position. Previous CentraCare employees qualify if they have not worked for CentraCare for at least 9 months at the date of hire. Relocation assistance requirements: Must accept a position that offers benefits and may not have worked for CentraCare in the last 9 months. Must currently reside more than 50 miles away from Waite Park and relocate within 30 miles of Waite Park. CentraCare hiring incentives to include but not limited to, hiring and relocation bonuses may be discontinued at any time.
