What will I do in the role of LPN Clinical Coordinator?
  • Assume the role as a Clinical Leader responsible for the daily supervision of PC staff and coordination of all nursing activities to ensure the highest quality of care.
  • Be a role model in attitude, management and leadership. Exemplify a high standard of customer service.
  • Observe and monitor quality of care given by nursing personnel and make recommendations for improvement through additional training or educational needs including the coordination of bi-annual CPR and DHS Diabetes Overview Class.
  • Develop and implement policies and procedures needed to ensure adequate and appropriate delivery of care and services within regulatory requirements, standard of practice and established healthcare philosophy.
  • Ensure that residents' medical records regarding annual physicals, laboratory studies, physician's orders and nurses' notes are maintained and in compliance with PCH regulations.
  • Oversee Staff Scheduler; responsible for personal care staffing to ensure 24 hour coverage. Work with Scheduler as needed to assure adequate staffing requirements. Serve as a GPS guide and review job performance, skills and competencies of team members; participate in employee disciplinary action when necessary.

What do I need in the role of LPN Clinical Coordinator?
  • Current licensure by the Pennsylvania State Board of Nursing Examiners.
  • Successful completion of the Medication Technician Train the Trainer certification course within the first 6 months.
  • 3+ years' nursing supervision experience in Personal Care or related field.
  • Complete Dementia Specialist certification within the first year of hire.
  • Must maintain a current CPR certification.
