Job description

Position Summary
This position provides direct primary care services by filling temporary staffing needs, supporting provider coverage, assisting with new clinic launches, and improving patient access throughout the region. The ideal candidate enjoys practicing in a variety of clinical settings, embraces team-based care, and is comfortable caring for patients across the lifespan, from infants through older adults. While not required, the ability to provide care for patients of all ages is strongly preferred. Bilingual a plus.
Essential Duties and Responsibilities
Clinical Care
  • Provide comprehensive primary care services consistent with evidence-based practice.
  • Diagnose, treat, and manage acute and chronic illnesses.
  • Perform preventive care including annual wellness visits, well-child exams, sports physicals, and immunizations.
  • Order, interpret, and communicate laboratory and diagnostic test results.
  • Prescribe medications and treatment plans within Louisiana licensure and organizational protocols.
  • Provide patient education promoting healthy lifestyles and disease prevention.
  • Coordinate specialty referrals and follow-up care.
  • Utilize telehealth services when appropriate.
  • Collaborate closely with physicians and interdisciplinary care teams.
Float Coverage Responsibilities
  • Travel to assigned Community Health Centers and School-Based Health Centers throughout the Northshore region.
  • Provide scheduled and unscheduled provider coverage for vacations, leave, vacancies, and increased patient demand.
  • Support same-day access initiatives.
  • Assist with opening new clinic locations or expanding clinical services.
  • Maintain flexibility in work schedules based on operational needs.
  • Adapt quickly to varying clinic workflows and patient populations.
School-Based Health Center Responsibilities
  • Provide primary care services to enrolled students.
  • Perform school physicals and sports physicals.
  • Diagnose and manage common pediatric illnesses.
  • Coordinate care with parents, guardians, school nurses, and behavioral health providers.
  • Support immunization compliance and preventive screenings.
  • Participate in school health education initiatives as needed.
Community Health Center Responsibilities
  • Provide comprehensive care for pediatric, adolescent, adult, and geriatric patients.
  • Manage chronic diseases including Diabetes, Hypertension, Asthma, COPD, Hyperlipidemia, Obesity, and mental health conditions within scope.
  • Perform women's health services within scope of practice.
  • Promote preventive screenings and wellness initiatives.
  • Participate in quality improvement and population health initiatives.
Team Collaboration
  • Work collaboratively with physicians, behavioral health providers, dentists, pharmacists, nurses, medical assistants, care coordinators, and referral specialists.
  • Support integrated behavioral health workflows.
  • Participate in interdisciplinary case conferences.
  • Serve as a resource for nursing and clinical staff.
Documentation
  • Document all patient encounters accurately and timely in the electronic medical record.
  • Maintain compliance with Access Health Louisiana documentation standards.
  • Ensure accurate coding and billing documentation.
  • Complete charts within required organizational timelines.
Quality & Compliance
  • Adhere to HRSA, CMS, Louisiana Department of Health, FTCA, OSHA, HIPAA, Joint Commission (if applicable), and organizational standards.
  • Support achievement of UDS clinical quality measures.
  • Participate in quality improvement projects.
  • Follow infection prevention and patient safety policies.
  • Maintain clinical competencies.
Minimum Qualifications
Master's or Doctoral degree from an accredited Nurse Practitioner or Physician Assistant program.
Licensure - Nurse Practitioner
  • Current Louisiana APRN License
  • Certified Family Nurse Practitioner (FNP-C or FNP-BC)

Experience
  • Required: Minimum two (2) years of primary care experience.
  • Preferred: FQHC, pediatric, family practice, school-based health center experience, and experience caring for patients across the lifespan.

Travel Requirements
This is a regional float position requiring regular travel between Access Health Louisiana Community Health Centers and School-Based Health Centers throughout the Northshore service area. Reliable transportation, a valid Louisiana driver's license, and proof of automobile insurance are required.
