Job Description:

  • Assess patient conditions and needs through comprehensive evaluations to develop individualized care plans.
  • Develop and implement patient care plans in collaboration with the interdisciplinary team, ensuring comprehensive and continuous care.
  • Administer medications and treatments accurately and safely according to prescribed protocols.
  • Monitor and evaluate patient progress continuously, adjusting care plans as necessary to ensure optimal outcomes.
  • Document patient care activities meticulously to maintain accurate and up-to-date medical records.
  • Educate patients and families about health conditions, treatments, and post-discharge care to promote understanding and compliance.
  • Advocate for patient safety by identifying potential risks and implementing preventive measures.
  • Collaborate with healthcare team members to ensure coordinated and effective patient care.
  • Maintain sterile environment and adhere to safety protocols to prevent infection and ensure a safe environment for patients and staff.
  • Perform therapeutic and diagnostic interventions such as IVs, wound care, medication administration, catheterization, and specimen collections.
  • Provide emotional and physical support to patients and their families during recovery.
  • Engage in quality improvement initiatives to enhance patient care quality, safety, and efficiency.
  • Performs other duties as assigned.
