Elite Home Health & Hospice is seeking compassionate and skilled Registered Nurses (RN) to join our growing Home Health and Hospice team. The Registered Nurse provides high-quality, patient-centered nursing care to individuals in their homes while promoting comfort, dignity, independence, and quality of life.

Position SummaryThe Registered Nurse (RN) is responsible for providing skilled nursing services, comprehensive assessments, care planning, patient education, coordination of care, and clinical oversight in accordance with physician orders, Medicare regulations, and agency policies.
The RN works collaboratively with physicians, therapists, social workers, chaplains, aides, and other members of the interdisciplinary team to ensure safe and effective patient care.
Essential Job Responsibilities
  • Perform comprehensive patient assessments and reassessments.
  • Develop, implement, and update individualized Plans of Care.
  • Provide skilled nursing care according to physician orders.
  • Monitor patient conditions and report changes promptly.
  • Administer medications and treatments as ordered.
  • Educate patients and caregivers regarding disease management, medications, and safety.
  • Coordinate care with physicians and interdisciplinary team members.
  • Participate in patient care conferences and IDG meetings as applicable.
  • Provide wound care, medication management, symptom control, and disease management.
  • Ensure accurate, timely, and complete clinical documentation.
  • Follow Medicare Home Health and Hospice regulations and Conditions of Participation.
  • Participate in quality improvement activities and agency compliance programs.
  • Provide compassionate support to patients and families.
Additional Hospice RN Responsibilities
  • Provide end-of-life care focused on comfort, dignity, and symptom management.
  • Complete hospice nursing assessments and ongoing evaluations.
  • Assist with pain and symptom management.
  • Educate families regarding hospice philosophy and plan of care.
  • Collaborate with the Hospice Medical Director and interdisciplinary team.
  • Participate in emergency and after-hours support when required.
  • Support patients and families through the dying process with compassion and respect.
QualificationsRequired Qualifications
  • Current Registered Nurse (RN) license in the State of Texas.
  • Graduate of an accredited nursing program.
  • Minimum one (1) year of nursing experience preferred.
  • Home Health, Hospice, Hospital, ICU, Med-Surg, or skilled nursing experience preferred.
  • Valid Texas Driver's License.
  • Reliable transportation and current automobile insurance.
  • Strong clinical assessment and communication skills.
  • Ability to work independently and manage a flexible schedule.
Preferred Qualifications
  • Previous Home Health or Hospice experience.
  • Oasis assessment experience for Home Health.
  • Hospice experience with symptom management and end-of-life care.
  • Knowledge of Medicare regulations and documentation requirements.
  • Experience with EMR systems.
  • Bilingual candidates are encouraged to apply.
