Position Summary:
Unison Health is seeking a registered nurse to provide comprehensive care to clients in community base. Responsibilities include conducting physical and mental health assessments, administering medications, providing medication education, and delivering direct nursing care. This position consists of one 8-hour day shift per week in an outpatient setting and four 8-hour day shifts in the Bowling Green community, supporting both Assertive Community Treatment (ACT) and outpatient clients. The ideal candidate is compassionate, organized, and committed to promoting the health and recovery of individuals with mental health and substance use needs.
Key Responsibilities & Role Highlights:
  • Openness to work in the community.
  • Maintain the Woodruff Outpatient Medication Room, including coordinating medication orders with the pharmacy, managing sample medications, and monitoring inventory and expiration dates.
  • Provide nursing services to outpatient and community-based clients, including medication administration, medication education, and ongoing health monitoring.
  • Conduct behavioral and physical health assessments, provide crisis intervention, and develop nursing care plans in collaboration with clients and the interdisciplinary treatment team.
  • Coordinate care and serve as a liaison between providers, clients, families, and community resources to ensure continuity of care and support treatment goals.
  • Provide emergency first aid as needed and maintain compliance with all agencies, state, and regulatory requirements related to nursing services.

Education & Experience Requirements:
  • Requires associate degree in nursing. Bachelor's degree preferred.
  • Previous psychiatric experience is preferred.
  • Experience working with SMI
  • This position requires current RN license from the State of Ohio. Certification as a psychiatric nurse is preferred.
  • Must demonstrate comfort in interacting with individuals with mental illness.
  • Must establish and maintain effective working relationships.
  • Must have excellent customer service skills.
