Job description

Join a dedicated Assertive Community Treatment (ACT) Team providing comprehensive support to individuals with severe and persistent mental illness who have not benefited from traditional clinic-based services. This role involves outreach, enrollment, and delivering community-based mental health and social services, including wellness self-management counseling, integrated dual disorder treatment, and daily living skills enhancement. The clinician works collaboratively with other treatment teams to offer intensive, person-centered care aimed at helping consumers maintain community living with appropriate resources.
Essential Functions:

  • Participate in daily team meetings to assign cases and review responsibilities.
  • Conduct comprehensive assessments to determine appropriate ACT services.
  • Utilize trauma-focused, evidence-based practices to address symptoms and improve emotional and functional well-being.
  • Develop crisis prevention plans and treatment goals in partnership with consumers.
  • Coordinate care with community partners, families, physicians, and other providers.
  • Support consumers in achieving treatment goals, medication education, crisis management, and daily living activities.
  • Monitor client safety as needed.
  • Complete ongoing diagnostic evaluations and required clinical assessments.
  • Participate in discharge planning and support transitions to community or higher levels of care.
  • Foster a person-centered, outcome-oriented program culture dedicated to continuous quality improvement.
  • Provide culturally competent services and complete all documentation and training timely.

Behavioral Skills:
  • Integrity, compassion, accountability, respect, and excellence in communication and leadership.

Education and Experience:
  • Master's Degree in Social Work, Psychology, or Marriage and Family Therapy required.
  • Preferred 3-5 years experience with severe mental illness or serious emotional disturbance populations.
  • Experience working with diverse and high-risk populations.

Licensure and Certification:
  • Licensed at Masters Level Clinician (BSRB) required.
  • Qualified Mental Health Professional (QMHP) training required.
  • Valid Driver's License.
  • Obtain temporary Masters Addiction Counselor licensure within 6 months and full licensure within 12 months of employment.
  • Preferred Clinical Level Licensure by BSRB and Clinical Addictions Counselor license.
