Job Description
  • Assess the member's current health status, resource utilization, past andย present treatment plan and services, prognosis, short and long term goals,ย treatment and provider options
  • Coordinate services between Primary Care Physician (PCP), specialists,ย medical providers, and non-medical staff as necessary to meet the completeย medical socio economic needs of clients
  • Develop plan of care based upon assessment with specific objectives, goalsย and interventions designed to meet member's needs
  • Provide patient and provider education
  • Facilitate member access to community based services
Compensation:ย  $27-$33/hr
Qualifications
  • MUST be an RN
  • 2 + years of Case Management experience from Home Health, Hospital, or nursing faciity
  • Must have excellent computer skills
  • Active driver'sย license
  • Managed Care Experience and Arizona Health Care Cost Containment System (AHCCCS).
  • Arizona's Medicaid agency PREFERRED but not required!
