RN Case Manager

Salary $34.00 to 40.00 per hour

Responsibilities of RN Case Manager:
  • Care Coordination: RN Case Managers are responsible for managing and coordinating the care of patients, particularly those with chronic or complex medical conditions. They work closely with patients, families, healthcare providers, and insurance companies to ensure appropriate care is delivered.

  • Assessment and Planning: Conduct comprehensive patient assessments to identify physical, psychosocial, and environmental needs. Develop and implement individualized care plans in collaboration with interdisciplinary healthcare teams.

  • Monitoring Progress: Monitor patient progress toward established goals and adjust care plans as necessary based on outcomes. Facilitate communication between patients, families, and various healthcare providers to ensure continuity of care.

  • Patient Advocacy: Serve as advocates for patients, ensuring their needs and preferences are taken into account throughout the care process. This includes educating patients about their treatment options and helping them navigate the healthcare system.

  • Resource Utilization: Identify and coordinate necessary medical appointments, transportation, and referrals to community resources. Ensure that healthcare resources are used efficiently.

Qualifications for RN Case Manager:
